As a   Internal Medicine Physician   on our interdisciplinary team, you’ll have the opportunity to form lasting, meaningful relationships with your patients and make a tangible difference in their lives every day.

Unlike traditional primary care settings, our   value-based PACE program   empowers you to provide comprehensive, integrated care focused on quality — not volume. With a smaller patient panel, you’ll be able to dedicate the time and attention each participant deserves.

What You’ll Do:

  • Conduct thorough, in-person physical and wellness assessments.
  • Collaborate with the Interdisciplinary Team to design and implement individualized care plans.
  • Coordinate participant transitions between acute, short-term, and long-term care settings.
  • Focus exclusively on holistic care — no hospital rounds — with an average of eight patients daily.

Requirements

  • Completion of an accredited residency program (ACGME required)
  • MD or DO degree
  • Board Certification preferred
  • Active, unrestricted California state medical license
  • State CDS certification and DEA registration
  • Eligibility to participate in Medicare and Medi-Cal programs
  • Specialties considered:   Internal Medicine, Family Medicine and Geriatrics
